Continuity of Care as Peoples Needs Change
Our range of services lets us provide continuity of care as peoples needs change over time, by having such a wide range of support services.
By providing a pathway of support people can reduce or increase support, with a consistent staff team who know their holistic and clinical needs well.
Many of the people we support have been deemed to have behaviours that are considered ‘too challenging’ for mainstream nursing and social care settings.
These individuals are now supported to receive high quality care and support tailored to their needs – ensuring they can stay within the community, reducing hospital and secure facility admission.
The Fieldbay teams are supported by a dedicated in-house multidisciplinary team that includes:

Fieldbay Supported Living Services in South Wales
Fieldbay provide support to people who have complex challenging needs, arising from a brain injury, mental health diagnosis, neurological conditions and often associated physical disabilities including epilepsy and associated health needs.
Their specialist supported living services are also designed for people to build community opportunities. Encouraging everyone to achieve their goals – tailoring support to meet individual needs, promote enablement and deliver outcomes.

Specialist Services – Brain Injury
Fieldbay take a positive approach to rehabilitation towards a full and active life for people with a range of traumatic and acquired brain injuries.
Brain Injury services focus on rehabilitation and development of people’s skills as they step down from hospital on the next step of their recovery journey.
The team at Fieldbay can offer a range of models including nursing and residential care and supported living services.
Focusing on strengths, highly trained staff work with people, their families and linked health and social care professionals to ensure continuation of treatment plans. With continued clinical input from the internal Multi Disciplinary Team people receive personalised support focussed on setting and achieving short, medium and long term goals.


Crusader Medical Care provide specialist nursing care to clients with complex care needs and/or challenging behaviours often ensuing from mental illness. They provide a holistic client-centred level of care, ranging from light intervention and life skill programmes to 24 hour 1:1 care. Crusader can accommodate a diverse clientele group from the ages of 18 years and upwards.

C.P.I Care Ltd is a specialist care provider that offers support to adults with learning disabilities and also those with mental health needs. As well as supported living, they also have a dedicated respite service and several development centres. Its team of in-house experts can create specialist packages of care within a supported living environment. Each package of care is designed with the individual at the centre – it is holistic in its approach and tailored to their needs. In addition to meeting the needs of the individuals, each package also identifies goals for development – either to improve skills or quality of life.
Quality Assurance
Services at Fieldbay meet conditions of registration under the Regulation and Inspection of Social Care (Wales) Act 2016.
The Huntington’s Disease Association is a registered charity which works to improve care and support services for people with Huntington’s. They educate families and professionals and champion people’s rights. The HDA sets a benchmark by which social care providers can be measured. Fieldbay’s specialist team in South Wales have developed close links with the association and are proud to be Quality Assured at their Yr Ysgol Service.
