TLC Care and Support provides Residential Care and Supported Living services to young people and adults aged 16 plus across Essex, Suffolk and Hertfordshire. Highly trained teams provide personal and enablement care so people can be empowered towards more independent living.

The team have developed a range of services supporting young people transitioning from living with family, moving from a children’s residential service or from residential school aged 16 plus.They ensure transition is a positive experience for young people and their families.
To ensure a successful transitional process for young people, we work in partnership with adult teams to create clear pathways.
Experienced teams explore options with young people and their families around housing, vocational opportunities, education or community activities.
Young people can move into a residential service, a bespoke supported living service or supported flat scheme.
Collaboration with young people, their families and linked health and social care professionals ensures the right placement and environment to meet needs.
Supported Living
The team support a range of social care needs including people with a learning disability, those on the autistic spectrum and associated complex challenging needs and/or a mental health diagnosis.
TLC’s supported living services are also designed for people to build community opportunities. Encouraging everyone to achieve their goals – tailoring support to meet individual needs, promote enablement and deliver outcomes.
The team can work with people at every stage – from assessing needs, finding individual service requirements, sourcing the right property and providing high-quality person-centred support.

Residential Services at TLC
Residential services at TLC provide personal and enablement care 24/7 so people can live as independently and safely as possible. Support empowers people towards more independent living.
The principle focus of all support offered is based on the ethos of ‘Positive Behaviour Support’ and ‘Person Centred Active Support’. Working with people to learn new skills, enable better communication and ultimately become more independent.
The team at TLC support people to move on from residential services when the time is right and onto more independence. Residential support keeps people safe, ensures that identified risks are managed well and empowers people to take part in new activities and experiences in line with wishes.
